Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards. On or about March 19, 2012, and at times prior thereto, employees were exposed to stuck-by hazards by catering trucks when employees walked to and from the outbound dock area, and across the driveway to the stage parking area. Among other methods, feasible means of abatement may include, but are not limited to the following: 1. Perform traffic control studies at the site integrating principles of motor vehicle safety with the goal of minimizing intersections of vehicle and pedestrians traffic. The study should address the configuration of the dock and parking area; installation of properly located signage for both vehicles and pedestrians; clear road markings; installation of properly located and designated pedestrian footpaths, walkways and crossings; coordination of truck and equipment movements; and illumination and noise interferences. 2. Provide training and information to affected personnel regarding outcomes and requirements of measures implemented as a result of the traffic control studies.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to slip and trip hazards. On or about March 19, 2012, the employer did not ensure adequate illumination was being maintained exposing employees to slips, trips and fall hazards at the following locations: a. at the outbound dock area, while employees utilize either the east or west stairways, b. at the stage parking area, while employees conduct their pre-check truck inspections. Among other methods, feasible means of abatement may include, but are not limited to the following: 1) Install adequate lighting in accordance with American National Standard Institute (ANSI) for Industrial Lighting RP-7, 1970.
