On or about November 15, 2011, employees were required to use a wire rope
for
Geronimo line that was kinked in two locations.  This condition exposed
employees to
a fall hazard.
Among other methods, one feasible and acceptable method to correct this
hazardous
condition would be to ensure that the wire rope used as an escape line is
removed from
service when evidence of kinking, crushing, cutting, cold working, or
bird-caging is
observed, in accordance with the American Petroleum Institute Recommended
Practice
for Occupational Safety for Oil and Gas Well Drilling and Servicing
Operations (API 54
