Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to laceration hazards: (a) Location - 75 Pleasant Street, Marblehead, MA: On or about 10/20/11, the employer failed to ensure that an employee was adequately instructed with how to avoid kickback from a chain saw. Among other methods, a feasible means to eliminate or reduce this hazard is to ensure that all workers are thoroughly trained in accordance with ANSI Z133.1-2006 (Safety Requirements for Arboricultural Operations) and the manufactures' operating instructions for a Dolmar series PS-5100 S chain saw.
