Labor and Employment Article, Section 5-104(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a)Date of Violation:On or About 01/12/12 Location:In Front of 450 William Street, Cumberland MD, at Curb Clean-Out Pipe Equipment:Vac-Con, Hot Shot Sewer Cleaner Truck, Model HS-1600 Condition:Employees working with a sewer cleaning hose and nozzle were exposed to struck by hazards due to using a "home-made" nozzle, not installing the "tiger tail", and not using a "skid", per the manufacturer's requirements. Among other methods, one feasible and acceptable means of abatement method to correct this hazard is to use the manufacturer's provided nozzles, "tiger tail" and "skid" to prevent the nozzle and hose assembly from propelling backwards towards the operator. Abatement documentation (photographs, receipts, copies of programs, training certification, etc.) is required for this item.

Among other methods, one feasible and acceptable means of abatement to correct this hazard is to use the manufacturer's safety manual procedures to provide employees with effective training. Abatement documentation (photographs, receipts, copies of programs, training certification, etc.) is required for this item.
