Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards while being elevated on the hydraulic loading dock: a) Loading dock Hydraulic loading dock not maintained as recommended in owner's manual exposing employees to crush by hazards. Violation disclosed on or about 12/22/2011. Among methods, all of the following are feasible and acceptable means of abatement: Schedule regular preventative maintenance by a qualified & responsible person to include lubrication, visual checks & operation checks at intervals described in the owner's manual.
