The employer did not furnish employment and a place of employment which
were free from
recognized hazards that were causing or likely to cause death or serious
physical harm to
employees in that employees were exposed to the hazard of falling from an
elevated pallet
attached to the forks of a forklift:
a) at the establishment - employees standing on an elevated pallet
positioned on the forks
of the Caterpillar (model #2FC15) electric forklift truck was exposed to
an 8'8" fall
hazard. The employees stood on the elevated pallet to retrieve paper from
the top shelf,
observed on or about 11/21/11.
Among other methods, one feasible and acceptable means of abatement is the
use of a
commercially manufactured safety basket that is specifically designed for
use on powered
industrial trucks and meets the requirements of the American Society of
Mechanical
Engineers' Safety Standard for Low Lift and High Lift Trucks (ASME
B56.1-2000).
