Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about 1/20/12 in the repair shop; Wheeltronic Ltd. (model 1776A - serial number T-923140 ) was a missing the safety release cable which attached to the slave side. Employer was using a screw driver to hold open the mechanical safety release devices when lowering vehicles exposing employees to crushing and struck by hazards. ABATEMENT DOCUMENTATION REQUIRED Feasible and acceptable abatement methods to correct this hazard: 1) Remove lift from service 2) Reinstall a new safety release cable 3) Follow Wheeltronic Ltd. Installation and Operation Manual: Inspect lift daily. Do not operate if it malfunctions of problems have been encountered. 4) Implement a lift inspection program as per manufacturer recommendations.
