Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were exposed to a crushed-by hazard: a.On or about 1/31/12 - Baler area, employees operated a Pro 5 Doosan forklift that was not equipped with a safety restraint system. Among other methods, one feasible and acceptable method to correct this hazard would be to ensure that all forklifts are provided with a restraint device and drivers utilize restraint devices as stated in ASME B56.1b-2003, Safety Standard for Low Lift and High Lift Trucks, Section 7.41, Operator Restraint Systems.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were exposed to a fall hazard: a.On or about 1/31/12 - Aluminum sorting area, an employee was exposed to a fall of approximately 7 feet while accessing and working on top of the scrap pile in a roll-off container. Among other methods, one feasible and acceptable method to correct this hazard would be to prohibit employees from entering the container in accordance with the manufacturer's warning label.g
