Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish a place of employment which was free from recognized hazards that
were causing or
likely to cause death or serious physical harm to employees, in that
employees were exposed
to fall hazards:
a) On or about 12/07/11 employees used a pallet attached to the forks of a
fork lift to
elevate workers to heights above ten feet to pick orders in excess of 120
lbs;
employees were not provided with fall protection.
Feasible and acceptable abatement methods to abate the hazards are to:
1. Utilize only platforms designed for elevating personnel with forklift.
These
platforms must meet the requirements set forth in ANSI/ASME B56.1-1993
such
as
but not limited to, fall protection for employees.
2. Train all employees on the hazards of falls associated with improper
elevating
methods and proper application and use of safety platforms.n
