release of anhydrous
ammonia in that: A) On or About 03/07/12 All piping (main, headers, and
branches)
were not properly labeled; B) On or About 03/07/12 The machine room was not
effectively sealed off from other areas of the plant, including a lack of
tight seals
where piping penetrated wall, spaces, and/or ceilings; C) On or About
03/07/12 The
emergency shut off switch was not located in an area separated from the
machine room
by a fire rated door; D) On or About 03/07/12 The control for the emergency
ventilation was not located outside of the machine room; E) On or About
03/07/12
pressure relief valves were not replaced every five years nor were they
labeled with
their in-service dates; F) On or About 03/07/12 piping and valves,
including
the
Pressure relief valves, were not labeled; G) On or About 03/07/12 The
discharge pipe
was not located at least 15 feet above any adjacent grades or roof levels;
H) On or
About 03/07/12 Potentially flammable and combustible materials were stored
within
the machine room; I) On or About 03/07/12 Ammonia detectors had not been
calibrated and maintained according to the manufacturers recommendations;
J) On or
About 03/07/12 Calculations were not available to verify that the
discharge piping
from each pressure relief valve was sized appropriately; K) On or About
03/07/12
That the ventilation provided the appropriate number of air changes per
hour, or that
the emergency ventilation would provide the appropriate number of air
changes per
hour; L) On or About 03/07/12 Written emergency shut down procedures had
not been
developed; M) On or About 03/07/12 Corrosion and vibration testing had not
been
conducted; and, N) On or About 03/07/12 an eyewash station was not found
working
in an adjacent room.

hazard is to comply with industry
recognized and generally accepted good engineering practices including,
but not limited to, those consensus
standards published by the American Society of Heating, Refrigerating and
Air-Conditioning Engineers
(ASHRAE), American National Standards Institute (ANSI), and the
International Institute of Ammonia
Refrigeration (IIAR).
Abatement documentation (photographs, receipts, copies of programs,
training certification, etc.)
is required for this item.
