Labor and Employment Article, Section 5-104(a): The employer did not
furnish employment and a place of
employment which were free from recognized hazards that were causing or
likely to cause death or serious
physical harm to employees:
(a)Date of Violation:05/04/12
Condition:Employees were not adequately protected against the potential
release of anhydrous
ammonia in that: A) On or About 05/04/12 All piping (main, headers, and
branches)
was not properly labeled; B) On or About 05/04/12 None of the valves
contained
within the piping system were effectively labeled; C) On or About 05/04/12
The
machine room was found to not have approved, self-closing, tight-fitting
fire doors
fitted with panic-type hardware and that these doors were not labeled with
approved
informative signs, emergency signs, charts and labels; D) On or About
05/04/12
The
system was not fitted with a remote emergency shut off control; E) On or
About
05/04/12 A dual pressure relief device located on the receiver and
approximately six
feet above the adjacent grade was found without a discharge pipe; F) On or
About
05/04/12 Ammonia detectors had not been calibrated and maintained
according to the
manufacturers recommendations; G) On or About 05/04/12 Calculations were
not
available to verify that the discharge piping from each pressure relief
valve was sized
appropriately; H) On or About 05/04/12 Calculations were not available to
verify that
the ventilation provided the appropriate number of air changes per hour,
or that the
emergency ventilation would provide the appropriate number of air changes
per
hour; I) On or About 05/04/12 Written emergency shut down procedures had
not been developed; J) On or About 05/04/12 An eyewash station and body
shower was not found located external to the machinery room and readily
accessible via an exit; K) On or About 05/04/12 A fuel-burning boiler
having
an
open flame was found to be located inside the machine room which permitted
combustion air from the machine room to be utilized by the boiler.
Among other methods, one feasible and acceptable method to correct this
hazard is to comply with
industry recognized and generally accepted good engineering practices
including, but not limited to:
those consensus standards published by the American Society of Heating,
Refrigerating and Air-
Conditioning Engineers (ASHRAE), American National Standards Institute
(ANSI), and the
International Institute of Ammonia Refrigeration (IIAR).
Abatement documentation (photographs, receipts, copies of programs,
training
certification, etc.) is required for this item.
