Labor and Employment Article, Section 5-104(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a)Date of Violation:On or About 03/20/12 Equipment:Liftall Forklift, Model #HTMS 100, Serial #DU6573 Condition:An employee did not utilize a restraint system while operating a forklift. Among other methods, one feasible and acceptable method to correct this hazard, utilize a restraint system in accordance with the current version of American National Standards Institute (ANSI) B56.1, "Safety Standard for Powered Industrial Trucks," and American Society of Mechanical Engineers (ASME) B56.1-2000, "Safety Standard for Low Lift and High Lift Trucks." Abatement documentation (photographs, receipts, copies of programs, training certification, etc.) is required for this item.
