General-duty citation text
HAR 12-60-2(a)(3) was violated because: A)Employees were not provided training in emergency response and rescue procedures. The lack of training exposed employees to potential serious injuries in the event of an accident that could occur while the employees are working aloft in the trees. (ANSI Z133.1-2006, Section 3.3.4) B)A job briefing was not performed by a qualified arborist in charge prior to the start of the job. The lack of a job briefing prevented an opportunity for structural defects in the tree involved in the accident to be identified prior to the employee climbing the tree and being exposed to serious injuries due to fall hazards. (ANSI Z133.1-2006, Section 3.1.4) 12-60-2(a)(3) states "Every employer shall provide safe work places and practices by elimination or reduction of existing or potential hazards. Elimination of existing or potential hazards by design, process substitution, or other appropriate methods is preferred because it eliminates the need for further employee protection. When elimination is not feasible, reduction of existing or potential hazards to acceptable levels, using methods such as engineering or administrative controls, isolation, or guarding, shall be promptly used. When these methods are inadequate to reach acceptable levels, personal protective equipment shall be provided and used." Location: Job sitesion:
