12-60-2(a)(3) was violated because: The employer did not provide safe work places and practices by elimination or reduction of existing or potential hazards. Employees were working in a warehouse that had shelving units with unanchored column legs and damaged support braces due to incidental contact with forklifts, exposing employees to struck by/crushed by hazards. Furthermore, the storage racks did not have visible load plaques showing the maximum permissible unit load per level. The damaged storage racks were allowed to maintain pallets of bulk products loaded/unloaded by forklifts, thus increasing the risk of employee exposure to physical injuries in the event of a collapse. ANSI MH16.1:2008, Specification for the Design, Testing, and Utilization of Industrial Steel Storage Racks, Section 1.4.2 states "Plaques shall show in clear, legible print (a) the maximum permissible unit load and/or maximum uniformly distributed load per level." Section 1.4.7 states "All rack columns shall be anchored to the floor with anchor bolts capable of resisting the forces caused by the horizontal and vertical loads on the rack." 12-60-2(a)(3) states "Every employer shall provide safe work places and practices by elimination or reduction of existing or potential hazards. Elimination of existing or potential hazards by design, process substitution, or other appropriate methods is preferred because it eliminates the need for further employee protection. When elimination is not feasible, reduction of existing or potential hazards to acceptable levels, using methods such as engineering or administrative controls, isolation, or guarding, shall be promptly used. When these methods are inadequate to reach acceptable levels, personal protective equipment shall be provided and used." Location: Staging area (ewa wall, diamond head, and ocean wall)
