Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing, or likely to cause, death or serious physical harm to employees in that employees were exposed to being struck by falling equipment: On Peeler Ranch well# 4, employees were preparing the rig for circulation operations. The crew supervisor did not ensure devices or equipment, such as, but not limited to, a 2-inch plug valve assembly were assembled and secured in a manner to prevent falling prior to elevating the piping and assembly 30 to 40 feet above the drilling floor. Among other methods, one feasible means to abate this hazard is to complete the assembly of devices and equipment at, or about, the level of the rig floor.
