Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were exposed to struck-by crushed-by hazards: (a)On or about 02/16/12 - Fabrication, Hyster Forklift, a safety seat belt was not used while the forklift was being operated. Among other methods, one feasible and acceptable method to correct this hazard is to utilize the attached safety seat belt and to train the operator in the safe operation of forklifts.
