a) for the employee(s) trimming palm tree branches located at 3555 SR 60 E
Mulberry (FL), as observed on or about 01/24/2012.  An employee was
working from a wooden pallet which had been elevated with the forks of a
Hyster RC150 forklift.  The employee was exposed to a fall hazard of eight
feet.
Abatement Note(s): Among other methods, means of feasible and acceptable
abatement includes but may not be limited to:
1. Use of a manufacturer's approved working platform designed for elevating
employees which is compliant with ANSI/ITSDF B56.1-2005 guidelines;
2. Use of ladder(s); and/or,
3. Use of pole saws designed for tree trimming.
