which were free from recognized hazards that were causing or were likely
to cause death or
serious physical harm to employees in that employees were exposed to:
On or about 02/03/12, at 451 Lincoln  Road Miami Beach FL, an employee was
struck by a
load of building material being placed into position for unloading from a
rough terrain forklift.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to have the employees working with and/or around the forklift, work in
accordance with ANSI
Standard B56.6 2005 that states in part that the operator understand the
rough terrain forklift
limitations and operate the truck in a safe manner as not to cause injury
to personnel.
PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS
VIOLATION.
