On or about 04/16/2012, an employee was exposed to a fall hazard of
approximately 16 feet
while working from a pallet on the forks of a Toyota forklift.
Among other means, a feasible method of abatement is to prohibit employees
from riding or
standing on the forks of the forklift and to utilize an a aerial lift to
elevate employees.
