North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from fire and explosion hazards: a)facility - where employees worked in near proximity to wood shavings which were being collected and blown through a cyclone and into in a surge bin that was partially constructed of plywood, which is a combustible material, (see reference 1). Abatement methods include removing the plywood and replacing it with a non-combustible material such as metal. b)facility, wood shaving dryer area - where employees worked in near proximity to the rotary-style wood shaving dryer that did not have automatic spark detection and extinguishing systems installed between the dryer drum and downstream material-handling equipment, such as the cyclone at shaker #2 and the cyclone housed at the wood shaving surge bin,(see reference 2). Abatement methods include installing a spark detection and extinguishing system between the rotary-style wood shaving dryer and the downstream equipment. c)facility, area behind bagging machine - where employees worked in near proximity to nonconductive ducts such as PVC pipes connected to a motor underneath the wood shaving surge bin which were used to remove dust and wood shavings from the air at the bagging machine, (see reference 3). Abatement methods include removing the PVC pipes and replacing them with pipes made of a conductive material such as metal or removing the current dust collection system and replacing it with a system designed and made with materials that would eliminate fire hazards. d)facility, wood shaving surge bin - where employees worked in near proximity to equipment (wood shaving surge bin, cyclone, and dust control motor under surge bin) designed for fugitive dust control which was not maintained and operated in a manner that minimized the escape of dust, (see reference 4). Abatement methods include performing maintenance on the current dust collection system to locate leaks/sources of where dust is escaping and repair the system, or replacing the current dust collection system with one that will minimize the fugitive dust emissions. Reference 1: National Fire Protection Agency (NFPA) 664 Standard for the Prevention of Fires and Explosions in Wood Processing and Woodworking Facilities (2012), paragraph 8.2.1.4, "A fire hazard shall be deemed to exist in the system wherever dry wood particulate is collected or conveyed or wherever components of the conveying system are constructed of combustible materials." Reference 2: National Fire Protection Agency (NFPA) 664 Standard for the Prevention of Fires and Explosions in Wood Processing and Woodworking Facilities (2012), paragraph 8.6.2.2, "Rotary dryers shall have automatic spark detection and extinguishing systems installed between the dryer drum and downstream material-handling equipment, such as cyclones or wind boxes." North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from fire and explosion hazards: a)facility, bagging machine area - where employees used compressed air to clean the beams, walls, floor, and equipment surfaces around the bagging machine (Verville VP-400) causing the wood dust to become airborne in the presence of multiple ignition sources (heat, electricity). Reference: National Fire Protection Agency (NFPA) 664, Standard for Prevention of Fires and Explosions in Wood Processing and Woodworking Facilities (2012), paragraph 11.2.1.1, "Surfaces shall be cleaned in a manner that minimizes the generation of dust clouds. Blowing down with steam or compressed air or even vigorous sweeping shall be permitted only if the following requirements are met: (1)The floor area and equipment shall be vacuumed prior to blowdown. (2) Electrical power and other sources of ignition shall be shut down, removed from the area, or classified for use only in dusty areas per NFPA 70, National Electrical Code. (3) Only a low gauge pressure of 103kPa (15 psi) steam or compressed air shall be used. (4) No open flames, sparks from spark-producing equipment, or hot surfaces capable of igniting a dust cloud or layer shall exist. (5) All fire protection equipment shall be in service.t
