Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were exposed to being caught in the point of operaton: (a)On or about 03/16/12 - Press #7, AIDA 150 Hydraulic Press, the two-hand control buttons were not guarded against unintentional actuation. Among other methods, one feasible and acceptable method to correct this hazard is to follow the instructions in ANSI B.11.2, 1995, Section 6.3.9(1) which states that when two-hand control is provided, each control shall be protected against unintended actuation. Also provide training to employees on press operation to include the two-hand control device.
