Title 40.1-51.1(a) Code of Virginia: At 14516-I Lee Road, Chantilly, VA,
on April 20th, 2012, the
employer, Total Mechanical Service Inc, did not furnish employment and a
place of employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm to employees
in that employees were exposed to a fall hazard when accessing the roof
when the aluminum extension ladder
did not reach above the roof edge.  An employee utilized an aluminum
extension ladder to access the roof of an
approximately 20 foot high building, but the top of the ladder was placed
well below the roof edge, requiring
the employee to grab onto the roof edge and pull himself up.  The
extension ladder was missing the
manufacturer provided safety and instructional labels.
One feasible and acceptable method, among others, to correct this hazard
is for the employer to follow the
American National Standard for ladders-portable metal-safety requirements
(ANSI A14.2-1990), which states in
section 8.3.10 "Access to Roof. When single section or extension ladders
are used to gain access to a roof, the
top of the ladder shall extend at least 1 foot and no more than 3 feet
above the point of support at the eaves,
gutter or roof line.  The ladder should be tied down to prevent slipping
before the user ascends onto the roof."
ANSI A14.2-1990 also states under section 9, Labeling/Marking
Requirements, 9.1.2 "Markings shall be
legible and readily visible." and 9.2.1 "The product data information
markings in 9.2.2 shall appear on the
outside of the lower left rail (climber facing the ladder) and where
possible, approximately 4-1/2 to 6 feet from
the bottom of the ladder."
