A.R.S. 23-403.A.:  The employer did not furnish to each of his employees
employment and a
place of employment which were free from recognized hazards that were
causing or likely to
cause death or serious physical harm to their employees, in that the
employer that employees
were operating three hydraulic automotive lifts for which they had not
received information and
training in the operation of the lifts and the lifts had not been
inspected on a regular basis as
required by the manufacturer and by the American Standards Institute
(ANSI) and the
Automotive Lift Institute, Inc (ALI) - ANSI/ALI ALOIM-2008 - "Standard for
Automotive
Lifts-Safety Requirements for Operation, Inspection and Maintenance" for
the following lifts
Western Hoist models:
#1 - Overhead Beam Style Lift, model WH9A, S/N 15531
#2 - Overhead Beam Style Lift, model TP9KAC, S/N 110723582
#3 - Overhead Beam Style Lift, model AB-1597, S/N 166
a) Body Shop:  The employees were not provided with information and
training
in the safe
operation of the lifts.
