Condition:An employee was elevated approximately 15 feet on the forks of a
material picker
while standing upon a pallet without fall protection. The pallet was not
secured to the
forks, allowing its displacement.
Among other methods, one feasible and acceptable abatement method to
correct this hazard would be to
prohibit the use of a material picker to lift personnel and obtain a
personnel platform designed to safely lift
personnel.
Abatement documentation (photographs, receipts, copies of programs,
training certification, etc.)
is required for this item.
