employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that employees were exposed to struck-by and/or crushing
hazards:
a)jobsite, on July 25, 2012, where an employee was working near or below
metal
plate connected wood trusses that were installed 17 feet above the ground
and not
adequately supported because the third bearing support was missing due to
the
incomplete interior masonry wall.
b)jobsite, on July 25, 2012, where an employee was working near or below
metal
plate connected wood trusses that were installed 17 feet above the ground
without
sufficient lateral restraint and diagonal bracing.
Among other methods, one feasible and acceptable abatement method would be
to follow the recommendations and guidelines for handling, installing, and
bracing wood trusses of HIB-91 from the Truss Plate Institute and the
booklet,
"Building Component Safety Information -- Guide to Good Practice for
Handling,
Installing, Restraining & Bracing of Metal Plate Connected Wood Trusses"
(BCSI) jointly produced by the Structural Building Components Association
and
Truss Plate Institute.
