11.5.1 NMAC Section 50-9-5(A), NMSA 1978 (New Mexico Occupational Health and Safety Act): Every employer shall frnish to each of his employees employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm to his employees: Employees are exposed to the hazard of working on a service rig that has not receved its mandatory Category III inspection with the last two years. The condition of the mast/derrick and substructure is unknown since the required in-depth visual inspection of load bearing components have not been accomplished. The Category III inspection is an industry standard as evidenced by the requirements of the American Petroleum Institute (API) Publication #4G, paragraphs 6.1, 6.2.3, 6.3 and 9.1. One feasible and accepted method includes but is no limited to contacting with a company having personnel with the qualifications described in API #4G, Section 4.2 needed to complete the inspection.

11.5.1 NMAC Section 50-9-5(A), NMSA 1978 (New Mexico Occupational Health and Safety Act): Every employer shall furnish to each of his employees employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm to his employees: Employees working on the service rig are exposed to the hazard of the controls having no or severely degraded labeling on the rig controls. While the rig operator is experienced, a moment of inattention or the need in an emergency for a less experienced hand to take a required action, could easily lead to confusion and delay. Clearly marked labels is an industry standard as evidenced by the requirements of the American Pertroleum Institute (API) Publication #4G, paragraph 9.2 and 9.2.1. One feasible and acceptable method includes but is not limited to the replacement of damaged labels as they occur. A label evaluation could additionally be made during the required Category III inspections. Date Corrected: Describe Corrective Action:
