Title 40.1-51.1(a) Code of Virginia: The employer, Independent Steel, LLC, did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck by/ caught between hazards while operating overhead chain hoists to lift materials between 500-600 lbs. In the Production area, five overhead chain hoists were supported by the steel web trusses for the roof of the building. The trusses were not marked with the rated load capacity, nor was there documentation showing that the trusses were designed for the purpose of supporting overhead chain hoists and any loads attached to the hoists, in addition to any additional loads placed on the roof (e.g., snow). One feasible and acceptable method, among others, to correct this hazard is for the employer to ensure the supporting structures of overhead hoists are designed to withstand the loads and forces imposed by the hoist for the rated load.
