At the Casino Monte Lago, the employer did not ensure that the workplace
was free
from recognized hazards when employees where allowed to work on a Genie
GS-1930
scissor lift that was missing the entry chain. This exposed employees to
fall hazards from
up to nineteen feet and one inch (19.1) while replacing can light bulbs at
the front
entrance of the casino.
