Nevada Revised Statue 618.375 Duties of employers. Every employer shall: 1. Furnish employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm to his or her employees. On February 4, 2013, Ted Wiens Tire & Auto Centers, located at 1201 South Rainbow, Las Vegas, NV 89108. The employer did not ensure the following processes were provided and practiced in accordance with ANSI/ALI ALOIM:2008: Reference: ANSI/ALI ALOIM-2000 Standard for Automotive Lifts - Safety Requirements for Training, Repair and, Annual Certification. Section 4.3 Operator Training Documentation: The owner or employer shall appropriately document operator training by completing the operator training log found in the appendix to this standard.Section 5.4 Qualified Inspection Documentation: A record of each periodic inspection shall be prepared and maintained noting the observations and findings of all pints of inspection recommended by the manufacturers as well as subsequent repairs or replacements accomplished. Section 5.5 Qualified Inspection Frequency: Inspections by the owner or employer shall follow the recommendations of the lift manufacturer as to frequency. Without regard to the frequency of inspection specified by the lift manufacturer, whether it be weekly, monthly, semi-annually or on some other basis, the owner or employer shall ensure that the points presented in 5.6 are inspected by a qualified lift inspector as a minimum annual requirement. Section 6.2.4 Repair Maintenance Documentation: A through record of each repair maintenance procedure performed shall be prepared and maintained by the owner or employer. Reference owner's manual (AMMCO LIFTS 6K LOW-RISE.) Page 7 - Lifting: (1) Hold toggle switch on pumping unit in ""ON"" position until locking leg drops into one of the three lifting height locking positions. Page 7 - Daily Inspections and Maintenance: (6) Check locking latch for smooth and proper operation. Reference Owner's Manual (WESTERN HOIST XLA-XCA-ARA.) Page 7 - Lifting: (4) Raise lift until automatic locking device is fully engaged. ONE FEASIBLE MEANS OF ABATEMENT WOULD BE TO MAINTAIN THE REQUIRED DOCUMENTATION AS STATED IN THE TRAINING, REPAIR, AND INSPECTION PROCEDURES IN ANSI/ALI ALOIM-2008 and the Owners manual.
