Nevada Revised Statute 618.375(1):  Duties of employers.  Every employer
shall furnish
employment and a place of employment which are free from recognized
hazards that are causing
or are likely to cause death or serious physical harm to his or her
employees:
At the Excalibur Hotel and Casino, located in the Engineering Department
repair shops,
inside the refrigerant tank storage room, the oxygen sensor was not
correctly maintained
exposing employees to possible serious hazards such as, but not limited to
tripping or
slipping from an employee experiencing dizziness, loss of coordination,
loss of
concentration, or death from asphyxiation, due to depletion of oxygen.
Reference: The employer, Excalibur Hotel and Casino, did not conduct
periodic
inspections of the oxygen sensor.  The employer did not maintain testing
of the alarm for
the oxygen sensor.  The oxygen sensor OS-1, located in the refrigerant
tank storage room
is about 20 years old and obsolete according to the manufacturer

One feasible and acceptable means of abatement method is to ensure that
the current
twenty year old oxygen sensor, model OS-1, located in the refrigerant
storage room is
replaced with a new oxygen sensor now and every five years in accordance
with the
manufacturer's recommendation. The employer needs to ensure employees
follow all
product information guidelines on the proper operation, inspection,
testing
schedules, and
maintenance tasks on the oxygen sensor.
