North Carolina General Statute 95-129(1) of the Occupational Safety and
Health Act of North Carolina:  The
employer did not furnish each of his employees conditions of employment
and a place of employment free
from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in
that employees were exposed to flammable gas explosion hazards:
a)facility, gas hydrogen system - the piping system was not marked with
the name of the gas and
direction of flow at each change of direction and at every 20 feet
throughout the piping run.
b)facility, gas hydrogen system - the emergency shutoff valves at the
point of use and at the bulk
source were not clearly marked.
c)facility, gas hydrogen system - signs were not posted in the area of the
flammable gas system
which stated that no smoking or open flame was prohibited within 25 feet
of the area.
ABATEMENT NOTE:
1)Among other methods, one feasible and acceptable abatement method to
correct these hazards
is to:
a) mark or label the piping system with the name of the gas and  direction
of flow at each
change of direction and at every 20 feet throughout the piping run
b)  keep vegetation at least 10 feet from the system
c)  mark or label the valves
d)  post signs which prohibit smoking or open flames in the area.
