Condition:An employee was elevated approximately 13 feet while standing on
a wooden pallet on
the forks of an order picker forklift while the pallet was not secured to
the lifting
carriage, and the employee was not protected from falling.
Among other methods, one feasible and acceptable method to correct this
hazard is to have the employee use a
personal fall arrest system as required by the forklift manufacturer.
Abatement documentation (photographs, receipts, copies of programs,
training certification, etc.)
is required for this item.
