North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to electrocution hazard as follows: a) Butterball farm #6, turkey houses - where Dayton 1YBU9, 3/4 HP, 1725 rpm, 115/230V, belt driven motors were used to operate 48" box style, cord and plug, agricultural fans; the strain relief and outer insulation on two of the motors were pulled away from the motor casing; exposing the thinly insulated internal wires to the sharp edges of the motor opening and allowing for dust and debris to enter inside of the motor casing. In one of these instances, the ground wire was exposed and visibly not connected to the motor. b) Butterball farm #6, turkey houses - where Dayton 1YBU9, 3/4 HP, 1725 rpm, 115/230V, belt driven motors were used to operate 48" box style, cord and plug, agricultural fans; the casing on one of the motors was broken, exposing thinly insulated internal wires to the sharp edges of the motor and allowing for dust and debris to enter inside of the motor casing. c) Butterball farm #6, turkey houses - where Dayton 1YBU9, 3/4 HP, 1725 rpm, 115/230V, belt driven motors were used to operate 48" box style, cord and plug, agricultural fans; the plug on one of the fans was being pulled away from the electrical outlet into which it was plugged, causing strain on the grounding pin. Abatement Notice: One feasible and acceptable means of abatement is to provide fans/motors which are wired and grounded in accordance with manufacturer recommendations and located such that there is no strain being placed on the motor end or plug end of the cord.
