A.R.S. 23-403.A.: The employer did not furnish to each of his employees employment and a place of employment which are free from recognized hazards that were causing or likely to cause death or serious physical harm to their employees, in that the employer did not follow the manufacturer's requirements for safe operation of automotive vehicle lifts: a) Quick Lane Service Line: A Rotary automotive lift, model SPO12N510, serial number BVR07H0009, was not removed from service when the lift's safety latch system was not operating properly and self-closing lift controls were overridden. b) Quick Lane Service Line: Employees had not been provided training on how to safely operate, maintain, and inspect automotive lifts prior to employees using the vehicle lift, in order to recognize and avoid unsafe conditions such as malfunctioning parts and inoperable safety devices.le
