Stafford, VA, the employer did not furnish employment and a place of
employment which were free from
recognized hazards that were causing or likely to cause death or serious
physical harm to employees in that
employees were exposed to the hazard of near infrared and far infrared
radiation exposure from Class IV
lasers: (a) On and before October 8th, 2013, laser technicians were
exposed to Class IV laser radiation hazards
while operating the ND:YAG 1064nm and Pixel YAG 2940nm modules connected
to the Alma Lasers,
Harmony XL.  Technicians were also exposed to Class IV laser radiation
hazards while operating the Alma
Lasers, Soprano XL, 810nm wavelength module.  Employees were not provided
adequate eye protection.
One feasible and acceptable method, among others, to correct this hazard
is for the employer to establish a laser
safety program in accordance with American National Standards Institute
(ANSI) Z136.1-2007, American
National Standard for the Safe Use of Lasers, and the manufacturer's
instructions,
including but not limited to:
(1) Appointment of a qualified Laser Safety Officer; (2) Conduct a hazard
safety analysis and establish safety-related standard operating
procedures; (3) Providing medical surveillance to employees; and, (4)
Provision of
safety training to technicians and other affected employees.
