Labor and Employment Article, Section 5-104(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a)Date of Violation:On or About 11/15/13 Location:Roof Peak of House Condition:Employees installing the vapor barrier and shingles on a 7/12 sloped roof were exposed to falls as great as 29 feet to ground when they utilized vertical lifelines as part of personal fall arrest systems that were tied and knotted around anchors at the peak of the roof. (b)Date of Violation:On or About 11/18/13 Location:Roof Peak of House Condition:An employee installing the vapor barrier and shingles on a 4/12 sloped roof was exposed to falls as great as 15 feet to ground when he utilized a vertical lifeline as part of a personal fall arrest system that was tied and knotted around an anchor at the peak of the roof. Among other methods, one feasible and acceptable method to correct the hazard is to comply with ANSI/ASSE A10.32-2004 Section 3 General Requirements: 3.7.3 Knots are not allowed in lifelines, lanyards or other direct-impact components. Abatement documentation (photographs, receipts, copies of programs, training certification, etc.) is required for this item.
