North Carolina General Statute 95-129(1) of the Occupational Safety and
Health Act of North Carolina:  The
employer did not furnish each of his employees conditions of employment
and a place of employment free from
recognized hazards that were causing or likely to cause death or serious
physical harm to employees in that
employees were exposed to a crushed by hazard.
a)Service Area:  where a Challenger (Model CL10V3, 10,000 lbs, 230V, 60Hz,
18A) automotive
lift was observed to be missing bolts on the extender arms and it and the
other lifts in the
service area including, but not limited to, ALM (Model 9002), Hunter
(Model RX12), Tuxedo
(Model TP11), Challenger (Model E10), and Ben Pearson (Model B9000A)
automotive lifts and
the employer could not verify that periodic qualified inspections of the
lifts had been conducted.
Reference:  ANSI/ALI 2008 Standard for Automotive Lifts - Safety
Requirements for
Operation, Inspection and Maintenance.  Section 5:  Periodic Qualified
Inspection - 5.1 "The owner or employer shall establish a periodic
inspection
procedure in accordance with the recommendations of the lift manufacturer
in
order to ensure reliability and allow the continued safe operation of the
lift."
5.6.2 Inspection Points - All Lifts:  5.6.2.12 "On lifts using swing arms,
check
to ensure that arm telescoping stops are functioning as intended.  Report
improper function or excessive wear.

Page 12 - Maintenance "The following maintenance points are suggested as
the
basis of a routine maintenance program.  The actual maintenance program
should be tailored to the installation.  See ANSI/ALI ALOIM booklet for
periodic inspection checklist and maintenance log sheet.  (Daily) "Check
for
loose or broken parts."
One feasible and acceptable method of abatement is for the employer to
periodically retain a
qualified lift inspection to conduct and document a periodic qualified
inspection
of each
automotive lift.otive
