Carolina:  The employer did not furnish each of his employees conditions

a place of employment free from recognized hazards that were causing or
likely to cause death
or serious physical harm to employees in that employees were exposed to
struck by hazards from
a damaged roof.
a)facility - employees working within the facility were exposed to struck
by hazards
while working underneath a roof system that had rotten wood.
ABATEMENT NOTE:
1)Among other methods, one feasible and acceptable abatement method is to
repair
the roof or remove employees from hazardous areas per Department of
Building
Safety and Code Enforcement.

hazards from forklift tipover.
a)facility -  employees operated a TCM brand, model FG10N16, an
Allis-Chalmers
brand, model ACP and a White brand, model MY60B, powered industrial truck
that were not equipped with seat belts.
ABATEMENT NOTE:
1)Among other methods, one feasible and acceptable abatement method to
correct
this hazard is to install seatbelts on forklifts.
