On and prior to January 7, 2014, in the Interior Re-Sale Storage area, steel storage racking systems were not installed or used properly. The storage racks were not anchored to the floor. The racks would be exposed to horizontal and vertical forces that could cause a collapse or tip over. This exposed an employee to the hazards of rack collapse or tip over. Employees were exposed to serious crushing injuries or death. Reference: ANSI MH16.1 - 2004(R2008) Specification for the Design, Testing & Utilization of Industrial Steel Storage Racks. Section 1.4.7: Column Base Plates and Anchors. The bottom of all columns shall be furnished with column base plates, as specified in section 7.2. All rack columns shall be anchored to the floor with anchor bolts capable of resisting the forces caused by the horizontal and vertical loads on the rack. One possible means of abatement would be to follow the design, and installation requirements set forth in ANSI MH16.1 - 2004(R2008).
