11.5.1. NMAC Section 50-9-5(A), NMSA 1978 (New Mexico Occupational Health and Safety Act): Every employer shall furnish to each of his employees employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm to his employees: Employees are exposed to the hazard of being struck by flying debris, or oil and gases due to well control not being maintained at all times, as was evidenced by two separate incidents at the Apache Lee Federal #87 well site. The method of controlling well pressure is an industry standard and is addressed in the American Petroleum Institute (API) Publication 54, paragraph 6.1.1. One feasible method to retain well control would be to install pressure gauge at the wellhead as well as downhole. Date Corrected: Describe Corrective Action:

11.5.1 NMAC Section 50-9-5(A), NMSA 1978 (New Mexico Occupational Health and Safety Act): Every employer shall furnish to each of his employees employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm to his employees: Employees are exposed to the hazard of being caught between the guy wires and the escape equipment due to the Geronimo line not being kept clear of obstructions at all times. The method of installing an auxiliary escape is an industry standard and is addressed in the American Petroleum Institute (API) Publication 54, paragraph 6.10.1. One feasible method to ensure that the auxiliary escape is correctly installed and unobstructed would be to conduct daily inspections of the equipment to ensure it meets the API standards.
