HAR 12-60-2(a)(3):  Every employer shall provide safe work places and
practices by
elimination or reduction of existing or potential hazards.  Elimination of
existing or potential
hazards by design, process substitution, or other appropriate methods is
preferred because it
eliminates the need for further employee protection.  When elimination is
not feasible,
reduction of existing or potential hazards to acceptable levels, using
methods such as
engineering or administrative controls, isolation, or guarding, shall be
promptly used.  When
these methods are inadequate to reach acceptable levels, personal
protective equipment shall
be provided and used.
Storage Castle facility at 304 Puuhale Road - The employer did not provide
a safe work
place by elimination of existing and/or potential hazards when moving
large, heavy items at
its establishment.  The employer had failed to implement a process
substitution where
mechanical means could have been used to handle and move large, heavy
items
instead of
having employees manually move these items.  On April 21, 2014, a
part-time helper, who
was working alone, was involved in moving large, heavy interior wall
panels out of a large
storage container.  The interior wall panels had shifted and tipped over
thus resulting in the
employee being struck and sustaining fatal injuries.
