practices by
elimination or reduction of existing or potential hazards.  Elimination of
existing or potential
hazards by design, process substitution, or other appropriate methods is
preferred because it
eliminates the need for further employee protection.  When elimination is
not feasible,
reduction of existing or potential hazards to acceptable levels, using
methods such as
engineering or administrative controls, isolation, or guarding, shall be
promptly used.
Warehouse Freezers 5,6,7,& 8 - The employer did not protect the Raymond
Easi Lift Stand
up forklift operators from under-ride or horizontal intrusion hazards.
On August 25, 2014, an operator of Raymond Easi Lift forklift, model
#EDR3011, serial
#T2A0121232, was killed when he was crushed between the dashboard of the
forklift and the
metal storage rack.
Feasible means of abatement include, but are not limited to:
1. Adjust the shelf heights so that horizontal members of the racks were
either level with the
canopy of the forklift and/or were significantly lower than the body of
the forklift.
2.  Install barrier curbing around the base of the storage racks.
3. Install a guard for the operator's station on the forklift.  Guards
installed on existing
equipment would have to be approved by the manufacturer (see ANSI.ITSDF
B56.1-2005,
para.4.5.3, 7.30, and 7.36).
