NCGS 95-129(1) section 5(a)(1) of the Occupational Safety and Health Act
of 1970:  The
employer did not furnish employment and a place of employment which were
free from
recognized hazards that were causing or likely to cause death or serious
physical harm to
employees in that employees were exposed to:
a)job site at 5771 Stonemill Dr., Building 5830, Winston-Salem,
NC-employee was
lifted along with roofing shingles to a height of approximately 30 feet to
the top
of a two story apartment building while standing on a wooden pallet placed
on top
of the forks of a JLG all terrain extendable boom lift model # 1044C-54,
United
Rentals # 1005785.
NOTE: Among other methods of abatement, one feasible and acceptable method
to correct this hazard is to prohibit the employees from using the machine
as
a
"man" lift or utility platform unless a proper man basket is used. Prohibit
employees from working from the forks or from a wooden pallet on the forks
while the forks are in the raised position of a JLG extendable boom lift.
