Condition:An employee was engaged in the mechanical mixing of Lehigh
portland
cement, a caustic with a pH of 12, while a Speakman self contained
portable eyewash was located 77 feet away from a concrete mixing area,
out of view, and without a sign marking its location and with the path
leading to the eyewash station obstructed by plastic wrap.
(b)Date of Violation:On or About 07/09/14
Location:Concrete Mixing Area
Condition:An employee was engaged in the mechanical mixing of Lehigh
portland
cement, a caustic with a pH of 12, while a Speakman self contained
portable eyewash was located 118 feet 6 inches away from a concrete
mixing area, out of view, and without a sign marking its location and with
the path leading to the eyewash station obstructed by plastic wrap.
(c)Date of Violation:On or About 06/25/14
Location:West Side of Building
Condition:An employee engaged in removing dried excess mortar from masonry
walls, using Sure Klean Vana-Trol, a corrosive with a pH of 0.3, while
a Speakman self contained portable eyewash was located 125 feet to 250
feet away from where employees were using the corrosive material to
clean bricks.
Among other methods, one feasible and acceptable abatement method of
abatement is for the
employer to install and locate the self-contained eyewash station so that
it is no more than 10
seconds travel distance from the hazard location to the eyewash, the path
of travel is not
obstructed, and the location of the eyewash is identified by a sign, in
accordance with American
National Standards Institute ANSI Z358.1-2009, Sections 5.4.2. and 5.4.3.
Abatement documentation (photographs, receipts, copies of programs,
training
certification, etc.) is required for this item.
