Labor and Employment Article, Section 5-104(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a)Date of Violation:06/30/14 Location:Lift Truck Charging Area Equipment:Crown Electric Powered Forklift, #33, Model #35 SCTT, Serial #W-71848 Condition: Employees who operated an electric forklift were exposed to possible ejection/crushing hazards due to the forklift not being equipped with a seatbelt. (b)Date of Violation:06/30/14 Location:Lift Truck Charging Area Equipment:Crown Electric Powered Forklift, #34, Model #35 SCTT-S, Serial #H-5178-2 Condition:Employees who operated an electric forklift were exposed to possible ejection/crushing hazards due to the forklift not being equipped with a seatbelt. (c)Date of Violation:06/30/14 Location:Lift Truck Charging Area Equipment:Crown Electric Powered Forklift, Model #35 SCTT-S, Serial #H-16235-4 Condition:Employees who operated an electric forklift were exposed to possible ejection/crushing hazards due to the forklift not being equipped with a seatbelt. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install and require the use the use of an operator restraint system in accordance with the American National Standards Institute (ANSI) B56.1-2005, Standard for Powered Industrial Trucks.
