Nevada Revised Statute 618.375(1):  Duties of employers.  Every employer
shall furnish
employment and a place of employment which are free from recognized
hazards that are causing
or are likely to cause death or serious physical harm to his or her
employees:
The employer failed to ensure that an employee was wearing fall protection
while
elevated to a height of approximately 25 feet.  An Archway employee was
observed on
the platform of a Crown Model 30SP36TT order picker at the 4th level,
approximately
25 feet off the ground, without fall protection.
One feasible and acceptable means of abatement is to conduct refresher
training
periodically to reinforce safety rules on operation of industrial trucks,
to include the
proper use of fall protection when elevated on the order picker industrial
truck.

Low Lift and High Lift Trucks, requires manufacturers to provide, and
operators to wear
operator restraint systems.
- Paragraph 4.17 (c) Elevating Personnel. Be certain that required
restraining means such
as railings, chains, cable, body belt(s) with lanyard(s), or deceleration
devices, etc., are
in place and properly used.
