29 CFR 1926.1417(c)(1): The procedures applicable to the operation of the equipment, including rated capacities (load charts), recommended operating speeds, special hazard warnings, instructions, and operator's manual, were not readily available in the cab at all times for use by the operator: a) Southeast exterior corner of structure; On or about 3/21/11 and at times prior thereto, the operator's manual for a Manitex truck mounted hydraulic crane (Model 30100) being utilized to hoist personnel was not available for use. b) Southeast exterior corner of structure; On or about 3/24/11 and at times prior thereto, the operator's manual for a Manitex truck mounted hydraulic crane (Model 30100) being utilized to hoist personnel was not available for use. 29 CFR 1926.1431(d)(1): When using suspended personnel platforms, the total load (with the platform loaded, including the hook, load line and rigging) exceeded 50 percent of the rated capacity for the radius and configuration of the equipment: a) Southeast exterior corner of structure; On or about 3/24/11 and at times prior thereto, the rated capacity for a Manitex truck mounted hydraulic crane (Model 30100) being utilized to hoist personnel was exceeded resulting in the crane tipping over. Note: Abatement certification and documentation are NOT required for this item.

29 CFR 1926.1431(h)(1): A trial lift with the unoccupied personnel platform loaded at least to the anticipated lift weight was not made from ground level, or any other location where employees would enter the platform, to each location at which the platform was to be hoisted to and positioned. a) Southeast exterior corner of structure; On or about 3/21/11 and at times prior thereto, two workers installing a sign were hoisted by a Manitex truck mounted hydraulic crane (Model 30100) without a trial lift being conducted. b) Southeast exterior corner of structure; On or about 3/24/11 and at times prior thereto, two workers troubleshooting/repairing a sign were hoisted by a Manitex truck mounted hydraulic crane (Model 30100) without a trial lift being conducted.
