29 CFR 1926.1428(a): The employer of the signal person did not ensure that each signal person met the Qualification Requirements (paragraph (c) of this section) prior to giving any signals. a) Southeast exterior corner of structure; On or about 3/21/11 and at times prior thereto, an unevaluated signal person was utilized for signaling via radio a Manitex truck mounted hydraulic crane (Model 30100) that was hoisting two employees in a man basket. b) Southeast exterior corner of structure; On or about 3/24/11 and at times prior thereto, an unevaluated signal person was utilized for signaling via radio a Manitex truck mounted hydraulic crane (Model 30100) that was hoisting two employees in a man basket. Note: Abatement certification IS required for this item.

29 CFR 1926.1431(h)(1): A trial lift with the unoccupied personnel platform loaded at least to the anticipated lift weight was not made from ground level, or any other location where employees would enter the platform, to each location at which the platform was to be hoisted to and positioned. a) Southeast exterior corner of structure; On or about 3/21/11 and at times prior thereto, two workers installing a sign were hoisted by a Manitex truck mounted hydraulic crane (Model 30100) without a trial lift being conducted. b) Southeast exterior corner of structure; On or about 3/24/11 and at times prior thereto, two workers troubleshooting/repairing a sign were hoisted by a Manitex truck mounted hydraulic crane (Model 30100) without a trial lift being conducted.
