29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person a)Advantage Acoustics, 751 Horizon Drive, Grand Junction, CO: The employer did not ensure that a "competent" person conducted an inspection of the worksite prior to beginning work. On or about April 4, 2011, employees were working on the second story of the motel without guardrails and/or Personal Fall Arrest Systems (PFAS). Condition exposed employees to a fall hazard of over 8 feet. b) A "competent" person did not ensured the use of hard hats by those working below others where material and/or equipment was capable of falling. On or about April 4, 2011, employees were working below others on the second floor of the motel without hardhats. Condition exposed employees to "struck" by hazards. Abatement notes: OSHA defines a "competent" person as one capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them.

Condition exposed employee(s) to "struck by" hazards from falling tools, material, and/or equipment. Advantage Acoustics was previously cited for a violation of this Occupational Safety and health standard or its equivalent standard 29 CFR 1926.100(a), contained in OSHA inspection number 110484672, citation number 1, item number 1, issued on August 14, 2006, with respect to a workplace located at 1601 Oxbow Drive, Montrose, Colorado. This citation became final on September 11, 2006. .
