29 CFR 1926.20(b)(2):  The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of the jobsite, materials, and equipment to be made by a Competent Person.    a)  Accurate Concrete Cutting, Inc., 751 Horizon Drive, Grand Junction, CO:  The employer did not ensure that a "competent" person conducted an inspection of the work environment prior to beginning work.  On or about April 4, 2011, employees were working on the second story of the motel without guardrails and/or Personal Fall Arrest systems (PFAS).  Condition exposed employees to a fall hazard of over 8 feet.    b)  A "competent" person did not ensure the use of hard hats by employees where overhead hazards existed.  On or about April 4, 2011, employee were working below others on the second floor of the motel without hardhats.  Condition exposed employees to "struck by" hazards.

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a)  Accurate Concrete Cutting, Inc. 751 Horizon Drive, Grand Junction, CO:  The employer did not ensure that fall protection was provided and used while employees were cutting concrete on the second floor of the motel renovation. Condition exposed employees of the employer to falls from the open sided floor and stairwell opening up to 8 feet 6 inches.

Condition exposed employees(s) to "struck by" hazards from falling tools, material, and/or equipment.
